- The Law we at Westminster follow:
- We are required to keep your health information private UNLESS you give us consent to share information with your spouse, family member, or friend. We may share your health information in the event of an emergency with those involved with your care.
- We ARE allowed to share your information when it involves your care and treatment, insurance claims, and business activities to improve the quality of care in our office. This includes researchers approved by an institutional review board.
- In partnership with Jefferson Center for Mental Health, a mental health provider is available at our office to provide mental health care to our patients. We may disclose patient health information, including mental health information, to/from Jefferson Center for Mental Health due to the nature of “integrated mental health services”. Disclosure of information includes written and/or oral communication.
- Your health rights:
- The health and billing records we maintain are physical property of Westminster Medical Clinic. The INFORMATION in the record belongs to you.
- At any time, you may request 1) a paper or electronic copy, an inspection of your records, 2) an amendment to correct incomplete information in your records, or 3) a restriction to use your health information.
- Our responsibilities:
- Maintain your privacy.
- Provide you with and abide by the Notice of Privacy Practices for Protected Health Information document.
- Notify you if we cannot provide you with or abide by the Notice of Privacy Practices for Protected Health Information document.
- Accommodate your reasonable requests. Such requests need to be in writing.
- To request information or file a complaint:
- We neither require you to waive your right to file a complaint nor retaliate against you for filing a complaint.
- If you need to file a complaint regarding how we handle your health information, please use the information in the Notice of Privacy Practices for Protected Health Information document.
