To ensure proper insurance billing for your visit, we require that we have your insurance information entered at the time of scheduling. Please be sure to have your insurance card available before scheduling online or calling our office. If you schedule online with a mobile device, you will have the ability to take a picture of your card front and back. You may also email us a copy of your card (FRONT AND BACK) to [email protected].
WMC accepts and bills most major health plans. The following are the ONLY insurance plans we are accepting as new patients at WMC.
We recommend you check with your insurance to make sure one of our Physicians is covered under your plan. If your plan requires you to select a Primary Care Physician (PCP) you will need to update that prior to your visit. If you fail to do so, your visit may not be covered and you may be responsible for the total cost of the visit.

- We are not a Medicaid/Health First Colorado provider.

What is the reason for my visit?
Many patients like to say "Annual Physical" or "New Patient Physical." or "Complete Physical." Most insurance plans cover a once a year "annual physical or annual wellness visit" with no out of pocket cost. These are for prevention only, NOT for discussion of current issues that must be evaluated and managed by your medical provider.
Evaluation and management of issues, questions and concerns, are billed differently than prevention, and are subject to copays, coinsurance, and/or deductible. If you need to discuss your concerns, need diagnostic labs or imaging, prescriptions sent or refilled (other than birth control), referrals sent that are not preventive in nature, this is an evaluation and management visit, and NOT an annual physical.
As a clinic, we prefer to address your current issues and concerns at the first visit, and schedule your annual preventive visit at a subsequent visit.
